Donor/Sponsor Form

Donations for raffle and silent auction items are key to the Courts for Courtney event. Items we are looking for include: sporting event tickets, sports memorabila, gift baskets, vacation packages, etc.

All proceeds will go to the Courtney Butcher Memorial Scholarship, a 501(c)3 non-profit fund. Contributions towards this event are tax deductible.

Cash, Checks and Visa/Master Card credit Cards will be accepted.
